Work/school physicals
Work and school physicals provide required medical evaluations, including vital sign assessments and completion of necessary documentation, to ensure patients meet health standards for employment, education, and activity participation in an efficient and compliant manner.
Lab draws onsite and point of care testing
Services include on-site blood draws, rapid testing for common conditions, and laboratory screenings as ordered by your provider. This allows for convenient, efficient evaluation and personalized treatment without the need for outside referrals.

Fayshalee Ortiz, DNP, APRN, FNP-BC
Fayshalee Ortiz, DNP, APRN, FNP-BC is a dedicated, board-certified Family Nurse Practitioner credentialed by the American Academy of Nurse Practitioners. She proudly serves patients at Sunrise Family Clinic, where she provides compassionate, patient-centered, and evidence-based care.
Originally from Jamaica, she began her nursing career at the Kingston School of Nursing, earning her Associate Degree in Nursing, and later completed her Bachelor of Science in Nursing at Northern Caribbean University. After relocating to the United States, she obtained her Master of Science in Nursing from the University of Texas Medical Branch in Galveston in 2014, followed by her Doctor of Nursing Practice degree from the University of Texas at Arlington in 2019.
Dr. Ortiz is passionate about caring for individuals and families across the lifespan and is committed to building trusting relationships with her patients. Guided by compassion, respect, and clinical excellence, she empowers patients with the knowledge and support they need to take an active role in their health while making a positive impact within the community she serves.
